Project PATH is an outreach program for Charles County residents who are homeless or are at risk of becoming homeless.  The program concentrates on men and women with mental health issues, some of whom may have problems with substance abuse.  We provide a variety of services including: outreach/case management, advocacy, assistance in applying for housing and applying for Supplemental Security Income (SSI).  We also provide links to services such as: healthcare, job opportunities, and education resources.


All potential clients must be Charles County residents with a diagnosed mental health condition. All referrals should come from any social service providing agencies within Charles County.
