Located in Rockville, MD, Chase Partnership House is a 36-bed transitional housing program serving single, homeless men who are struggling with addiction. Chase Partnership House provides comprehensive case management services and connects residents to substance abuse and mental health counseling, education and employment programs, and other community resources so that they may gain the skills and confidence needed to rebuild their lives and obtain permanent housing. Through the implementation of Individual Service Plans and a strengths-based approach, clients are empowered to address barriers to self-sufficiency and successfully transition from crisis and isolation to stability and independence
